Client needs prompting/support with the following:-
Bathing & personal grooming
Companionship
Help keeping active
Light housework
Meal preparation
Medication reminders
Running errands
Trips out of the house
Medication
Client has been in respite whilst his home has been renovated and it is important that these routines are mirrored once he returns home with regular prompting and encouraging him to maintain independence so that he can manage his activities of daily living.
Client's home will be newly refurbished. Client has an allotment nearby on which he grows vegetables. Also enjoys being in the garden when the sun is out. Client needs to be engaged with activities and socialisation as this will help maintain his speech. Client enjoys walking.

Care Requirements - Previous Condition Experience Needed by CAREGiver
Client has muddled speech and is not always coherent with what is being discussed. Client has dysphagia and has word finding difficulties and needs time to express himself.
Client needs to given time to allow him to complete tasks in his own time so as to not feel pressure whilst building a positive rapport with his live in carers.
2-3 weeks on and 2-3 weeks off.
Male carers are required. During the clients first week home the client and live in carer will also have the support of another CAREGiver during the day.

Care Requirements - Previous Equipment Experience Needed by CAREGiver
Client does not need any equipment at home however telecare equipment with alarm sensors to alert front and back doors being opened for example will be in place.
Client to live in a clean and safe environment.
Client to be encouraged to eat and drink and to have a healthy lifestyle promoted. Live in care to cook meals that meet the clients needs and encourage him to choose meals that he will eat.
Client is continent and manages this but may need guidance and this will help reduce falls risk particularly until client is used to his newly refurbished and renovated home.
Client needs to be prompted and guided through personal care routines, including supervision and prompting when dressing and undressing due to not always sequencing these tasks and risks of falls.
Client to be actively encouraged to make his own meals and drinks if needed.
Carer to undertake housekeeping tasks.
Additional Information
Client is male and aged 74 and will be returning to live in his newly refurbished and renovated home which may take him a little while to settle into.
Client has no family and is supported by solicitors. During Live In Carers downtime each day client will be supported by another CAREGiver for these times.
