Job Title: Social Worker – Hospital Discharge – Liverpool

Pay Rate: £43.78 per hour
Liquid Personnel is recruiting a Social Worker for the Adults Hospital Discharge Team in Liverpool. Candidates must have SWE registration, UK work eligibility, a degree in Social Work, and post-qualified experience.
The role involves conducting assessments and developing support plans for individuals being discharged from mental health hospitals. It requires working within a multidisciplinary team (MDT) to ensure coordinated care, safeguarding individuals by identifying and managing risks, and supporting recovery and independence through person-centred planning. Additionally, the role includes collaborating with partner agencies to facilitate safe and effective discharges.
