An exciting role has become available at the London Borough Of Lambeth Council. They are looking for skilled and experienced social workers to work in Transforming Care in the Adult Learning Disabilities Team in Lambeth Adult Social Care.
What will your responsibilities be?
In this role, you will work in the Transforming Care Team with adults living with learning disabilities and/or autism, who are at risk of special inpatient admission. You will aim to avoid admission through robust support planning and MDT collaboration and to expedite discharge to the community for existing inpatients.

Qualifications and Experiences:
You must have Social Work England registration and be eligible to work in the UK. Additionally, you need to hold a full UK driving licence and possess a degree level qualification or equivalent in Social Work.
